Facial fillers are materials that are injected under the skin to improve the wrinkles and abnormal scars. The most common area injected is the face and currently the most common injected materials are the synthetic (man made) fillers in comparison to natural substances such as fat. One size does not fit all. Experience in performing such procedures is essential for achieving the desired results.
Fat injection is the most common natural material used and is usually taken from another part of the patient’s body, processed and injected into the desired location. One of the problems with fat injection is its high rate of absorbsion and migration. It is a useful process in conjuction with other facial procedures such as facelifting and eyelid surgery. It is a somewhat more time consuming process and as a result it is not as popular as the other fillers.
Collagen was formerly the most popular injectable material. It is present all over the body, from skin down to and including bone and blood vessels. The main source is from highly purified bovine (cow) collagen. Zyderm and Zyplast are products that contain Lidocaine as an anesthetic and are injected under the skin. Zyplast contains more collagen and is used to treat deeper wrinkles. Since these products are derived from a non human protein, skin testing to detect sensitivity and allergy is required prior to treatment. Longevity is about 3 to 4 months. CosmoDerm and Cosmoplast are human derived collagen products produced with dermal fibroblasts harvested from bioengineered human skin cells. Cosmoplast is more effective for the treatment of deeper wrinkles. No skin testing is needed since these products have no animal proteins.
Hyaluronic Acid Fillers:
Hyaluronic Acid fillers are now the most popular and widely used fillers. They last longer than collagen and no skin pre testing is required. Hyaluronic Acid is present in our skin and is the one of the components in normal tissues. Its volume and quantity diminishes with time.
Restylane, Perlane & Prevelle Silk
Restylane, Perlane, and, Prevelle Silk are products that are used to treat wrinkles around the mouth, nasolabial folds, cheeks, acne scars, and frown lines with Perlane being used in deeper wrinkles. Restylane is used in the lips for increased fullness.
Juvederm Ultra & Juvederm Ultra Plus
Juvederm Ultra and Juvederm Ultra Plus are also Hyaluronic acids used to improve wrinkles and may last longer and up to a year for the Ultra Plus form. The XC type includes Lidocaine.
Radiesse is a filler composed of smooth calcium hydroxylapatite particles similar to the compounds found in bones and teeth, and suspended in a gel. It may have a more lasting effect since it stimulates an increased buildup of collagen for up to two years.
Poly(L-lactic) acid (PLLA):
Sculptrais a product that contains poly (L-lactic) acid based formulation which causes a gradual foreign-body reaction stimulating production of new collagen. It is approved by the FDA to restore facial volume loss seen in patients with HIV. Several sessions are needed and in general it is more painful that other injections. It needs to be repeated on the average every two years.
